08 Consents & authorizations Please read each statement carefully.
Photo & media consent. I grant Beyond the Bell permission to take photographs and video of my child for program documentation, newsletters, social media, and promotional use. I understand I may revoke this consent in writing at any time.Liability waiver for physical activity. I acknowledge that my child will participate in indoor/outdoor physical activities, organized sports, and recreational play, and I assume the risks inherent in such activities. I release Beyond the Bell, its staff, and partner facilities from liability for any injury that may occur during normal, supervised participation.Authorization for medical treatment. In the event of a medical emergency and if I cannot be reached, I authorize Beyond the Bell staff to obtain emergency medical care for my child, including transport by ambulance to the nearest hospital. I authorize trained staff to administer basic first aid and any medication listed above as prescribed.Field trip / off-site activity consent. I grant permission for my child to participate in supervised off-site activities and field trips associated with the program.Pick-up policy. I understand only people listed in section 05 may pick up my child, and that valid ID will be required at every pick-up until staff are familiar.Family handbook. I agree to read and follow the program's family handbook, including behavior, attendance, and payment policies.Accuracy of information. I certify that all information provided in this enrollment is true and accurate to the best of my knowledge.
